Ohio’s Hospice has developed Focused CareTM, a specialized program available from our care team, that addresses the disease-specific needs of the individual patient.
Working with the patient’s medical providers, our multidisciplinary team closely monitors the patient’s status and quickly responds to adverse symptoms as they occur.
Focused Care services are provided as part of the hospice benefit at no additional cost to the patient.
The goals of Focused Care include:
Oncology: Specialized oncology nurse available to work with you and your healthcare provider.
Cardiac: Specialized cardiac nurse to advise your care team and healthcare provider.
Pulmonary: Respiratory therapists and support for oxygen needs.
In many of our locations, Ohio’s Hospice offers an array of services that include hospital-based dedicated beds and Hospice House inpatient care. When patients require short-term, 24/7 symptom monitoring and management by a team with palliative and hospice expertise, these options for care are invaluable.
