Outstanding members of the Ohio’s Hospice staff are recognized every quarter with the Bouquet of Recognition Awards for going above and beyond in advocating for and serving patients. 

Carnation Award — Kari Diemunsch

Kari Diemunsch, executive administrative assistant and governance liaison at Ohio’s Hospice, was recognized with the Carnation Award. 

With a smile on her face, Kari handles so many situations with patience and grace. She has a can-do attitude and always ends a conversation with “Let me know if there is anything else I can do.” 

Whether she is planning Ohio’s Hospice’s semiannual affiliate leadership meetings, ordering key items or coordinating schedules of multiple teammates for a meeting, she repeatedly does this thoroughly and with kindness.

DAISY Award — Jackie Tims

Jackie Tims, nursing care manager at Ohio’s Hospice of Dayton, was recognized with the DAISY Award for her kind heart and gentle spirit. As she cares for her patients and families, she works hard to ensure that their last days are the best they can be. 

Cards and letters of recognition have been received from her patients and families, all expressing their appreciation for her kind, compassionate and competent care. Jackie knows what it means to carry out our mission because she does it daily, by lifting the spirits of a dying patient or supporting the spouse or child in need of comfort or understanding. She is a light to them when the world around them grows dim and dark. 

Jackie is changing the world for one patient and one family at a time. We appreciate her answering the call to service, and for carrying on the mission humbly and with a servant’s heart.

We honor Jackie for her hospice heart and thank her on behalf of every patient’s life she has touched with light and love.

Sunflower Award — Chauntay Berryman

Chauntay Berryman, personal care specialist at Ohio’s Hospice of Dayton, was recognized with the Sunflower Award for her exemplary patient care and hospice heart. She was nominated by her teammates, who consider her an amazing asset to their team. 

“Chauntay is exceptional in so many ways. She is calm, quiet, patient, hard-working, conscientious and kind,” one of her teammates wrote in the nomination form. “She lovingly cares for each patient on her assignment like they are the only one she has to care for.”

Another teammate thanked her for her dedication to her patients, and a third teammate praised Chauntay for her kindness. “I appreciate the kindness and care you provided the patient you were with today,” the teammate wrote in the nomination form. “She looked so happy when you took her for a walk.”

We are so honored to have Chauntay as a member of our care team. Her dedication and passion for patient care reflects the highest commitment to our hospice mission. Thank you, Chauntay.

Tulip Award — Jane Brewer

Ohio’s Hospice recognized Jane Brewer, chaplain at Ohio’s Community Mercy Hospice, with the Tulip Award. 

Jane responded to a call regarding a patient who has experienced a troubled life. The patient was struggling and needed spiritual support. 

Jane responded immediately and was able to bring her unique kindness and compassion to the patient. The patient was extremely thankful for Jane’s visit.
