Ohio’s Hospice Recognizes Staff With Bouquet of Recognition Awards for Fourth Quarter of 2022
Outstanding members of the Ohio’s Hospice staff are recognized each quarter with Bouquet of Recognition Awards for Excellence. Honorees personify the qualities found in the mission and values of Ohio’s Hospice and are the foundation upon which a remarkable patient experience is built. These staff members consistently demonstrate excellence through their expertise and extraordinary compassionate support for patients and families. They are recognized as outstanding role models in the Ohio’s Hospice community. These are the recipients for the fourth quarter of 2022.
Carnation Award
Jen Foley, community outreach liaison at Ohio’s Hospice of Dayton, was awarded the Carnation Award for spreading holiday cheer this past December at area assisted living facilities and extended care facilities. She met with residents and staff, bringing joy and smiles to each person she met.
As a community outreach liaison, Jen often meets with families. “Each day that I get the opportunity to work with families fills my cup,” Jen said. “But there is something else that warms my heart when I make a person smile because I brought joy to them.”
Danielle Thomas, team leader at Ohio’s Hospice of Dayton, nominated Jen for the Carnation Award. “She is amazing! She went above and beyond to spread holiday cheer and smiles throughout these facilities,” Danielle said. “She brings laughter and joy everywhere she goes. What an amazing individual!”

DAISY Award®
Teri Carver, RN Hospice House at Ohio’s Hospice of Dayton, was recognized with the DAISY Award® for providing exceptional patient care.
Ohio’s Hospice is proud to be a DAISY Award partner, recognizing its nurses with this special honor quarterly. At Ohio’s Hospice, DAISY Award honorees are nurses who consistently demonstrate excellence through their clinical expertise and extraordinary compassionate care.
Teri was nominated for her role in supporting a patient in attending his daughter’s wedding.
Teri was caring for a patient whose goal was to see his daughter get married. On her day off, Teri came in with all the things to make the wedding a success in only a few days.
“She embodies the hospice heart with each patient. She has an uncanny ability to tell things like they are while remaining professional and compassionate,” said Ashley LaVenture, RN Hospice House/IPU, who nominated Teri for the award. “Whether by lending a helping hand to her team or advocating for a patient, Teri truly goes above and beyond for every patient in every shift.”

DAISY Award®
Gayle Fleetwood, RN care manager at Ohio’s Hospice of Central Ohio, was recognized with the DAISY Award® for providing exceptional patient care.
Gayle was nominated by Machelle Arnold, program manager at Ohio’s Hospice of Central Ohio, for her dedication to the hospice mission and positive attitude through the changes in her role.
When the COVID-19 pandemic began, Gayle took on the new challenge to provide stellar care to her patients and grew her caseload to 17 patients, in addition to assisting with admissions and extended hours visits. Gayle has displayed a positive attitude through all the changes, and she is assisting her team with training on how to do admissions in the facilities.
“I appreciate Gayle’s leadership and her outstanding outreach to the other departments in our organization,” Michelle said. “Thank you, Gayle, for not only serving our patients and families but for serving your colleagues in the organization.”

Tulip Award
Janae Davidson, medical social worker at Ohio’s Hospice of Dayton, was nominated for the Tulip Award by a patient’s family for the compassionate care provided to their loved one and to the family.
The Tulip Award honors chaplains, social workers, therapists, counselors, physicians, pharmacists, and other patient care professionals at Ohio’s Hospice.
When a patient was in his final day, Janae took time out of her day off to visit the family and reassure them through the hard moments of the day.
“Janae cared for my father-in-law as he was passing. She didn’t hesitate to drop everything she was doing and come to the family in their time of need on her day off,” the family member said. “On that day, I saw an angel in action. My family and I are eternally in her debt. She was an oasis of relief.”
The patient’s family shared moments of extraordinary care provided by Janae that went above and beyond.
“She got me to take down my walls. I’ve never let myself deal with anything, but Janae was able to get through to me,” another family member said. “The day my father passed, Janae left her own family dinner to come be with us. I can say very strongly that I will be OK, and she is part of that.”
